If you are a NEW CUSTOMER of AppleJo Farms, this form is required to be completed before completing your Free Wait List  or $200 Deposit/Reservation request. 

Here at AppleJo Farms, bringing Goat Kids into the world and doing all we can to insure they will survive and thrive, is our ultimate goal. 

To accomplish this, it is important to help new customers be prepared for what is needed when taking kids to their new farm home.
